It is important that you read this carefully.
If you have any questions, please contact your referring doctor before your procedure.
Please bring the following to your Pre-Admission visit:
Health fund book and/or card
Medicare card
Pension card / Veterans Affairs card
Referral letter
Reading glasses, if appropriate
A list of current medications
Please return admission form to the hospital 14 days prior to admission, or as early as possible.
