It is important that you read this carefully.
If you have any questions, please contact your referring doctor before your procedure.
Please bring the following to your Pre-Admission visit:
- Health fund book and/or card
- Medicare card
- Pension card / Veterans Affairs card
- Referral letter
- Reading glasses, if appropriate
- A list of current medications
